About the Role
BAYADA Home Health Care is seeking a dedicated Occupational Therapist to join our team in providing high-quality home health care services. As an essential member of our multidisciplinary team, you will be responsible for delivering rehabilitative care and skilled nursing to clients in their homes throughout Hudson County.
Key Responsibilities
- Evaluate patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Develop individualized intervention plans to achieve favorable outcomes, taking into account cognitive, psychological, and motor aspects of performance.
- Assess and revise occupational therapy care plans regularly, ensuring continuity of care and collaboration with nursing as necessary.
- Educate and instruct patients, family members, or other patient representatives on rehabilitative care and activities necessary to promote independence.
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
