BAYADA Home Health Care is a leading provider of home health care services, seeking an experienced Occupational Therapist to join our team in Proctor, Vermont.
About the Role
- This is a full-time opportunity for a skilled OT to provide high-quality care and support to adult and geriatric patients in their homes throughout Rutland county and surrounding areas.
- The successful candidate will be part of a multidisciplinary healthcare team, working closely with clients, families, and other healthcare professionals to deliver personalized care and rehabilitation plans.
As a Home Health Occupational Therapist, you will have the opportunity to make a real difference in the lives of your clients, helping them to achieve their goals and regain independence in their daily lives. You will work collaboratively with clients and their families to identify and address any physical, cognitive, or emotional barriers that may be impacting their ability to perform daily activities.

You will also require strong organizational and time management skills, as well as the ability to work independently and as part of a team.
Responsibilities
- Evaluate patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Develop individualized intervention plans to achieve favorable outcomes, taking into account patients' goals, needs, and preferences.
- Educate and instruct patients, family members, or other patient representatives on rehabilitative care and activities necessary to promote independent living.
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
Requirements
- A current Vermont Occupational Therapist license.
- Prior home care experience preferred, but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills, organizational skills, and problem-solving skills.
