- Conduct thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Develop and implement individualized intervention plans to achieve favorable outcomes.
- Evaluate home environments for hazards or barriers to independent living.
- Identify equipment needs and environmental adaptations to enhance patient outcomes.
- Educate and instruct patients, family members, or representatives on rehabilitative care and activities necessary for optimal health, safety, and independent living.
