As a Home-Based Rehabilitation Professional, you will work closely with a multidisciplinary team to provide high-quality, patient-centered care to clients in the comfort of their own homes. Key Responsibilities
- Conduct home visits to assess client needs and develop personalized care plans.
- Perform diagnostic tests and measurements to evaluate client mobility, strength, and balance.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
- Educate clients and family members on rehabilitative care techniques and strategies for independent living.
- Accurately document client observations, interventions, and evaluations using state-of-the-art technology.
Requirements
- Current licensure as a Physical Therapist (PT) in Maryland.
- Graduation from an accredited Physical Therapy program.
- Strong communication, interpersonal, and problem-solving skills.
- Prior experience with electronic medical records (EMR) systems preferred.
