About the Role
We are seeking a skilled Home Health Physical Therapy Professional to join our team as a Physical Therapist Home Health Care Specialist. As a member of our multidisciplinary healthcare team, you will be responsible for delivering high-quality rehabilitative care to adult and geriatric clients in their homes.
Your primary focus will be on assessing client needs, developing and implementing personalized care plans, and providing one-on-one therapy sessions to promote physical function, mobility, and overall well-being.
In this role, you will work closely with other healthcare professionals to ensure comprehensive care that addresses the unique needs of each client. Your expertise and compassion will make a meaningful difference in the lives of our clients, enabling them to receive the medical care required to remain at home.
Key Responsibilities
- Assess clients' physical abilities and develop targeted care plans to address their specific needs and goals.
- Deliver high-quality physical therapy services, including manual therapeutic exercises, gait training, balance, and other interventions, to improve clients' functional ability and quality of life.
- Educate and instruct clients, family members, or caregivers on proper techniques and strategies for promoting physical function and preventing further injury or complications.
- Maintain accurate records of client interactions, progress, and treatment outcomes, utilizing a state-of-the-art touch pad tablet to facilitate seamless communication and collaboration with the interdisciplinary care team.
