About the Role:
BAYADA Home Health Care is seeking an experienced occupational therapist to join our team as a per diem home health care professional. As an integral member of our multidisciplinary healthcare team, you will be responsible for providing high-quality rehabilitative care and skilled nursing services to adult and geriatric patients in their homes.
Key Responsibilities:
- Conduct thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Develop individualized intervention plans to achieve favorable outcomes and continually assess and revise the occupational therapy care plan as necessary.
- Educate and instruct patients, family members, or other patient representatives on rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
Requirements:
- A current VA Occupational Therapist license.
- Prior home care experience preferred, but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
