About the Role
This position involves working as a Registered Nurse (RN) on our Home Health team, providing direct patient care and collaborating with healthcare professionals to achieve optimal outcomes.
Key Duties and Responsibilities:
- Conduct thorough assessments of clients' physical, emotional, and social needs
- Develop and implement individualized care plans that address identified needs and goals
- Administer medications, perform treatments, and provide wound care as prescribed
- Monitor clients' conditions and report changes to Clinical or Client Services Manager
- Document all interactions, assessments, and interventions in accordance with agency policies and regulations
