BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in providing high-quality care and services to clients. The ideal candidate will have at least one year of prior clinical experience and be able to make a positive impact on patients' lives.
Responsibilities:
- Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Continually assess and revise the Physical Therapy - PT plan of care, participating in multidisciplinary care plan meetings when appropriate.
