Job Description:
We are seeking an experienced Outpatient Physical Therapist to join our team in Chester Springs, Pennsylvania. As a skilled rehabilitation specialist, you will be responsible for providing personalized care to adult and geriatric clients in their homes, promoting independence, and improving overall quality of life.
Responsibilities:
- Make outpatient visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements to assess client needs.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and balance interventions.
- Continually assess and revise care plans as needed, collaborating with the nursing team to ensure comprehensive care.
- Educate and instruct clients and family members on rehabilitative care and activities necessary for independent living.
Requirements:
- A current PA Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
