About the Role
BAYADA Home Health Care is seeking a highly skilled and compassionate Home Health Care Professional to join our team as a Full-Time Outpatient Physical Therapist.
This rewarding opportunity will allow you to make a meaningful difference in the lives of geriatric clients residing in Assisted Living and Independent Living Communities in Newton and Wayland.
You will be instrumental in optimizing function and independence for older adults through proactive evidence-based interventions, working closely with a multi-disciplinary health care team that provides outpatient services to clients affording them the opportunity to age in place.
Key Responsibilities
- Making outpatient visits to clients in designated senior living communities.
- Performing diagnostic tests and measurements to assess client needs.
- Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assessing and revising the Physical Therapy plan of care, participating in multidisciplinary care plan meetings as appropriate.
- Educating and instructing clients, family members, or community staff on therapy services and activities necessary to promote client health, safety, and independent living.
Requirements
- A current Massachusetts Physical Therapist license.
- Previous experience as an outpatient Physical Therapist is preferred but not required.
