Job Description

Key Responsibilities
- Develop and implement personalized care plans for each client, taking into account their unique needs and circumstances.
- Conduct regular home visits to assess clients' conditions, provide necessary care, and document observations and interventions.
- Collaborate with clinical teams to ensure effective communication and seamless client care.
- Manage time effectively, prioritizing tasks and responsibilities to meet client needs.
- Document all interactions, treatments, and evaluations using a state-of-the-art touch pad tablet.
Requirements
- A current license as a Registered Nurse in MD.
- A minimum of one year of recent, verifiable clinical experience.
- Prior home care experience preferred, but not required.
- Graduation from an accredited nursing program.
- Strong interpersonal and communication skills.
