Job Overview
BAYADA Home Health Care is seeking a skilled and compassionate Outpatient Physical Therapist to join our team. As a key member of our multidisciplinary healthcare continuum, you will provide essential services to clients in independent living and assisted living communities.
About the Role
- Make outpatient visits to clients in designated senior living communities in the Loveland area.
- Supervise Physical Therapy Assistants as necessary or applicable, in accordance with state licensure requirements and professional standards.
- Attend and contribute to patient care, staffing conferences, and other related meetings.
- Makes referrals to other disciplines to address comprehensive client needs.
Responsibilities
- Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assess and revise the Physical Therapy plan of care, participating in multidisciplinary care plan meetings, as appropriate.
- Educate and instruct clients, family members, or community staff on therapy services and activities promoting client health, safety, and independent living.
Requirements
- A current CO Physical Therapist license.
- Outpatient experience preferred but not required.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
