BAYADA Home Health Care is seeking a highly skilled Physical Therapy Support Professional to join our team in Kapolei, Hawaii.

The successful candidate will have excellent communication skills and be able to work effectively with clients and their families. They will also have strong problem-solving skills and be able to think creatively to address the unique needs of each client.
Key Responsibilities:
- Assist clients in achieving their optimal physical wellbeing by providing treatment, tests, and client education as delegated by a licensed Physical Therapist.
- Implement appropriate individualized care plans, including therapeutic exercises, gait training, balance, and other interventions.
- Educate and instruct clients, family members, and other representatives on rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
- Accurately document observations, interventions, and evaluations pertaining to client care management, utilizing a state-of-the-art touch pad tablet.
Qualifications and Requirements
- A two-year college level program approved by the American Physical Therapy Association or equivalent experience.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
