Job Description
We are seeking a skilled and compassionate Registered Nurse (RN) to join our team in South Yarmouth, Massachusetts. The ideal candidate will have experience in home health care and be able to work independently with minimal supervision.
Responsibilities
- Develop and implement individualized care plans to meet the unique needs of each client
- Conduct regular home visits to assess client condition and provide medical treatment as needed
- Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination
- Document all client interactions and medical information accurately and thoroughly
Requirements
To be considered for this role, you must have:
- A current RN license in the state of Massachusetts
- At least 1 year of experience in home health care or a related field
