Job Description
We are seeking an experienced Physical Therapist to join our team as a Home Health Physical Therapist. As a member of our multidisciplinary health care team, you will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes. Your primary responsibilities will include making home visits to assess client needs, developing and implementing individualized care plans, and educating clients and their caregivers on proper rehabilitation techniques.
Required Skills and Qualifications
To be successful in this role, you must have a current Maryland Physical Therapist license, graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, and strong interpersonal, organizational, and problem-solving skills. You should also be able to work independently and manage your time effectively, with a focus on providing high-quality patient care.
