About the Role
We are seeking a skilled Physical Therapist to join our team as a Home Health Care Specialist. As a Senior Physical Therapy Clinician, you will work with adult and geriatric clients in their homes, providing one-on-one care and rehabilitation services. Your primary goal will be to help patients achieve optimal health and functionality, enabling them to live safely and independently at home.
Responsibilities
- Make home visits to clients, assessing their physical needs and developing personalized care plans.
- Perform diagnostic tests and measurements to evaluate client mobility, strength, and balance.
- Implement evidence-based interventions to promote wound healing, improve range of motion, and enhance overall function.
- Collaborate with nursing staff to develop and implement comprehensive care plans.
- Educate clients and family members on proper rehabilitative techniques and strategies for maintaining independence.
Requirements
- A current Pennsylvania Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
