About the Role
We are seeking a skilled Physical Therapist to join our team in providing high-quality care to our clients in their homes. As a member of our multidisciplinary healthcare team, you will be responsible for conducting home visits, assessing client needs, and developing individualized care plans.
Key Responsibilities
- Conduct thorough assessments of client mobility, strength, and balance
- Develop and implement individualized care plans, including manual therapeutic exercises and gait training
- Provide education and instruction to clients and their families on rehabilitative care and activities
- Document observations, interventions, and evaluations accurately and thoroughly
Requirements
- A current Ohio Physical Therapist license
- A minimum of one year of recent work experience as a Physical Therapist
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Strong interpersonal, organizational, and problem-solving skills
