About the Job: We are seeking a skilled Physical Therapist to join our team in Enosburg Falls, Vermont. As a home health care provider, you will work closely with adult and geriatric clients on a per visit basis, assessing their physical needs and developing individualized care plans to promote rehabilitation and independent living.
Responsibilities:
- Conduct comprehensive assessments of clients' physical abilities and develop targeted interventions to improve mobility, strength, and balance.
- Create personalized care plans that address clients' unique needs and goals, including manual therapeutic exercises, gait training, and other evidence-based practices.
- Educate clients and their families on safe and effective self-care techniques, promoting independence and confidence in daily activities.
- Maintain accurate and detailed records of client interactions, progress, and outcomes, using state-of-the-art touch pad tablets.
Requirements:
- A current Vermont Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Strong interpersonal, organizational, and problem-solving skills, with excellent communication and documentation abilities.
