About the Role
We are looking for an experienced Physical Therapist who can work independently and manage time effectively. As a home health care specialist, you will be responsible for making home visits to clients in designated geographic territories, performing diagnostic tests and measurements, and implementing individualized care plans.
Responsibilities
- Conduct thorough assessments of clients' physical abilities and develop personalized care plans to address their needs.
- Make regular home visits to clients, providing one-on-one therapy sessions and educating them on how to perform exercises and activities safely and effectively.
- Collaborate with other healthcare professionals to ensure comprehensive care and support for clients.
- Maintain accurate records of client interactions, progress, and any changes to their care plan.
Requirements
- A current Pennsylvania Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
