Job Description:
BAYADA Home Health Care is seeking an experienced Transitional Care Manager (TCM/Home Health Liaison) to join our Pikake Home Health division.
Key Responsibilities:
- Develop relationships with referral sources, physicians, and other healthcare professionals to promote BAYADA services.
- Market and sell BAYADA services by clearly articulating their features and benefits.
- Demonstrate expertise in clinical and regulatory home care eligibility criteria.
- Review medical documentation and collaborate with physicians to determine clients' eligibility for home care services.
- Coordinate with the service office to set up ordered services.
- Complete weekly referral logs.
Qualifications:
- Bachelor's degree or equivalent experience in the healthcare industry.
- Strong interpersonal skills and demonstrated knowledge of Medicare COPs and other program requirements.
- At least one year of experience in healthcare, preferably home health care.
- Previous discharge planning experience is preferred.
