About the Role
We are seeking a skilled Registered Nurse to provide high-quality home health care services. As a home health nurse, you will be responsible for making visits to clients in designated geographic territories and performing assigned duties, including administering medication, wound care, treatments, and procedures.
Responsibilities:
- Develop and implement individualized care plans based on patient needs
- Conduct thorough assessments of patients' conditions and report changes to Clinical or Client Services Manager
- Administer medications, perform wound care, and provide treatments as prescribed by physicians
- Document all observations, interventions, and evaluations pertaining to client care management and services provided
Requirements
To be successful in this role, you must have:
- A current license as a Registered Nurse in PA
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Prior home care experience is strongly preferred but not required
