Job Summary:
The ideal candidate will have a strong background in clinical care, excellent communication skills, and the ability to work independently. As a member of our team, you will be responsible for delivering exceptional patient care, collaborating with interdisciplinary teams, and contributing to the growth and development of our employees.
Key Responsibilities:
- Assess patient needs and develop individualized care plans
- Provide high-quality care to patients in their homes, including administering medications, performing wound care, and conducting home visits
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
- Document patient progress and maintain accurate records
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements:
- A current license as a Registered Nurse in NJ
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Prior home care experience strongly preferred, but not required
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
- Ability to travel to cases as assigned
