Job Description
- Administer medications, wound care, treatments, and procedures in the clients' homes.
- Monitor clients' conditions, reporting any changes or concerns to our clinical leadership team.
- Develop and implement comprehensive care plans in collaboration with our multidisciplinary team.
- Communicate effectively with clients, families, and caregivers to ensure seamless care coordination.
Qualifications
- A current license as a Registered Nurse in PA.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience is strongly preferred but not required.
- Graduation from an accredited and approved nursing program.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills and excellent communication abilities.
