Job Description
We are seeking compassionate and dedicated Registered Nurses to join our team to perform home visits to clients in designated geographic territories. The successful candidate will follow a designated care plan in accordance with patients' needs, administer medication, wound care, treatments, and procedures as necessary. They will also monitor clients' conditions, report changes to Clinical or Client Services Manager, and accurately document observations, interventions, and evaluations pertaining to client care management and services provided. Key responsibilities include case management and coordination, executing doctors' orders, and performing client assessments as necessary.
Required Skills and Qualifications
To be successful in this role, candidates must possess a current license as a Registered Nurse in Pennsylvania, a minimum of one year of recent clinical experience, and prior home care experience is strongly preferred but not required. Graduates from accredited nursing programs are encouraged to apply. The ideal candidate will have strong interpersonal skills, solid computer skills, and prior experience with electronic medical records (EMR). Ability to work independently and manage time effectively is essential.
