Job Overview
We are seeking a skilled and dedicated Registered Nurse (RN) to join our team as a Home Health Nurse Specialist. As a Home Health Nurse Specialist, you will provide one-on-one skilled nursing, intervention, and supportive care to patients in their homes.
Key Responsibilities
- You will see patients through their care needs, from hospital discharge to safe home life with comfort, independence, and dignity.
- You will deliver individualized care, including medication administration, wound care, treatments, and comfort care.
- You will perform patient assessments, educate patients and/or their family, and monitor patients' condition.
Requirements
- An active and current license to practice as a professional registered nurse (RN) in Pennsylvania.
- A minimum of one year of clinical experience.
- Previous home healthcare experience is welcome, but not required; we will train you.
- The ability to travel to assigned cases.
- A calm and reassuring presence, active listening skills, excellent assessment skills, and strong written and verbal communication.
