Responsibilities:
Requirements:
- Develop and implement comprehensive care plans tailored to individual patient needs
- Conduct thorough home visits to assess clients' conditions and provide high-quality care
- Administer medication, perform wound care, and conduct treatments and procedures as prescribed by physicians
- Maintain accurate and detailed records of client interactions, including observations, interventions, and evaluations
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
- Ensure compliance with regulatory requirements and BAYADA's policies and procedures
Requirements:
- Current licensure as a Registered Nurse in New Jersey
- Minimum one year of recent clinical experience
- Prior home care experience preferred but not required
- Graduation from an accredited and approved nursing program
- Strong interpersonal and communication skills
- Solid computer skills, particularly experience with EMRs
