- Case Management: Coordinate care and services, working closely with patients, families, and healthcare teams to ensure seamless transitions and optimal outcomes.
- Qualifications and Requirements: Possess a current license as a Registered Nurse in Pennsylvania; have a minimum of one year of recent, verifiable clinical (medical/surgical) experience; prior home care experience is strongly preferred but not required.
