We are looking for highly skilled and compassionate registered nurses to join our team of healthcare professionals.

Responsibilities

1. Patient-Centered Care: Provide comprehensive care to patients, following a designated care plan tailored to their unique needs.
2. Home Visits: Make regular visits to clients in designated geographic territories, ensuring timely and personalized attention.
3. Clinical Expertise: Administer medications, perform wound care, treatments, and procedures as required, maintaining the highest standards of clinical excellence.
4. Reporting and Documentation: Monitor clients' conditions, reporting any changes to Clinical or Client Services Managers, and accurately documenting observations, interventions, and evaluations using state-of-the-art touch pad tablets.
5. Case Management: Coordinate care and services, working closely with patients, families, and healthcare teams to ensure seamless transitions and optimal outcomes.
6. Qualifications and Requirements: Possess a current license as a Registered Nurse in Pennsylvania; have a minimum of one year of recent, verifiable clinical (medical/surgical) experience; prior home care experience is strongly preferred but not required.
