About the Job
We are seeking a skilled Registered Nurse to join our team in providing home health care services. As a member of our multidisciplinary team, you will be responsible for performing home visits, assessments, and admissions with adult patients. This is a full-time opportunity that requires excellent communication skills and the ability to work independently.
Responsibilities
- Perform home visits and assessments to determine patient needs
- Develop and implement individualized care plans to meet patient goals
- Administer medication, wound care, treatments, and procedures as prescribed by physicians
- Follow up with and execute doctors' orders accurately
- Document observations, interventions, and evaluations using state-of-the-art technology
Requirements
To be considered for this position, you must have a minimum of one year of recent clinical experience. Prior home care experience is strongly preferred but not required. You should also possess solid computer skills and experience with electronic medical records (EMR). Travel to cases as assigned is a requirement of this role.
