Job Summary: We are seeking an experienced Occupational Therapist to join our team as a home health occupational therapist. <1. Assess patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
<2. Develop individualized intervention plans to address cognitive, psychological, and motor aspects of performance and enhance outcomes for clients.
<3. Evaluate home environments for hazards or barriers to independent living and recommend necessary adaptations.
<4. Continuously assess and revise the occupational therapy care plan, collaborating with nurses and other healthcare professionals when necessary.
<5. Educate patients, family members, or caregivers on rehabilitative care and activities to promote optimal health and safety.
<6. Accurately document observations, interventions, and evaluations related to patient care management and services provided using state-of-the-art technology.
**Requirements include:**
* Current Vermont Occupational Therapist license
* Prior home care experience preferred but not required
* Minimum one year of recent work experience as an Occupational Therapist
* Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
* Ability to work independently and manage time effectively
* Strong interpersonal skills, organizational skills, and problem-solving skills
* Solid computer skills, with prior experience with electronic medical records (EMR) a plus
