Job Summary
We are seeking an experienced Occupational Therapist to join our team of healthcare professionals. This is a full-time opportunity to provide outpatient home health visits to adult and geriatric patients in senior living facilities throughout Ocean County, NJ.
Responsibilities
- Evaluate patients to develop a plan of care for functional independence
- Operate at top licensure utilizing current research-based approaches to care
- Work with the multidisciplinary healthcare team to transition care from home health to outpatient services
- Educate clients, families, and caregivers on care plans
- Perform evaluations, treatments, and discharge planning
Qualifications
- NJ Occupational Therapist license
- Prior home care experience preferred
- Minimum one year recent work experience as an Occupational Therapist
- Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
- Ability to work independently and manage time effectively
- Strong interpersonal skills, organizational skills, and problem-solving skills
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
