About the Role
We are currently seeking an experienced Occupational Therapist, OT, for a Full-time opportunity performing Outpatient home health visits for our Atlantic County Senior Living office located in Egg Harbor Township, NJ. This office services adult and geriatric patients on a per visit basis in Atlantic County, NJ.
The ideal candidate will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
Responsibilities
- Evaluate patients in designated senior living communities or group homes for adults with intellectual disabilities to develop a plan of care.
- Utilize current research-based approaches to maximize functional independence for clients.
- Closely work with home health therapy partners to transition care from home health to outpatient services to achieve client-centered goals in their living environment.
- Educate clients, families, and community caregivers on care plans to achieve a team approach to maximizing functional independence.
- Perform evaluations, treatments, and proper discharge planning using an outpatient EMR platform to deliver and document individualized treatment approaches.
Qualifications
- A current NJ Occupational Therapist license.
- Prior home care experience preferred but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills, organizational skills, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
