Job Description
BAYADA Home Health Care seeks a skilled Occupational Therapist to deliver high-quality care to adult and geriatric patients in their homes.
About the Role:
- Perform thorough evaluations to develop individualized plans of care for clients.
- Collaborate with home health therapy teams to ensure seamless transitions between services.
- Educate clients, families, and caregivers on care plans to promote functional independence.
- Document all treatments and progress using our outpatient EMR platform.
Requirements:
- NJ Occupational Therapist license
- Prior home care experience preferred but not required
- At least 1 year of recent work experience as an Occupational Therapist
- Graduation from an accredited occupational therapy program
- Strong interpersonal and organizational skills
- Solid computer skills and prior EMR experience preferred
