Job Description
BAYADA Home Health Care is seeking a skilled Occupational Therapist to provide home health visits in the Baltimore area.
This position offers the opportunity to work with adult and geriatric patients on a per-visit basis, serving communities throughout West County, including Reisterstown, Randallstown, Owings Mills, Pikesville, Windsor Mill, and Gwyn Oak.
Responsibilities:
- Make home visits to clients in designated territories, assessing their functional status and developing individualized intervention plans.
- Evaluate home environments for hazards or barriers to independent living, identifying equipment needs and environmental adaptations.
- Continually assess and revise care plans, participating in multidisciplinary care planning as needed.
- Educate and instruct patients, family members, or representatives on rehabilitative care and activities necessary for optimal health and safety.
- Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
Requirements:
- Current Maryland Occupational Therapist license.
- Prior home care experience preferred, but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
- Ability to work independently and manage time effectively, with strong interpersonal skills, organizational skills, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
