Job Responsibilities
As a Home Health Occupational Therapist, you will be an integral member of a multi-disciplinary healthcare team providing rehabilitative care to adult and geriatric patients in their homes.
Your responsibilities will include:
- Making home visits to clients in designated geographic territories.
- Performing thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Assessing cognitive, psychological, and motor aspects of performance and developing individualized intervention plans to achieve favorable outcomes.
- Evaluating the home environment for hazards or barriers to independent living and identifying equipment needs and environmental adaptations.
- Continually assessing and revising the occupational therapy care plan, participating with nursing in the multidisciplinary care plan as appropriate.
- Educating and instructing patients, family members, or patient representatives in rehabilitative care and activities necessary to promote health, safety, and independent living.
- Accurately documenting observations, interventions, and evaluations pertaining to patient care management and services provided.
Qualifications
To be successful in this role, you will need:
- A current MD Occupational Therapist license.
- Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
- The ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
