We are seeking an experienced Occupational Therapist to join our team in Durham, North Carolina. This full-time opportunity involves performing home health visits for our Raleigh office, serving adult and geriatric patients in Wake and Durham counties.
As a Home Health Occupational Therapist, you will be part of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing to clients, enabling them to receive medical care at home.
Responsibilities:
- Conduct home visits to assess patients' functional status, including muscle function, endurance, visual coordination, communication skills, and activities of daily living (ADL) performance.
- Develop individualized intervention plans to achieve favorable outcomes.
- Evaluate the home environment for hazards or barriers to independent living.
- Identify equipment needs and environmental adaptations for enhanced outcomes.
- Continuously assess and revise the occupational therapy care plan, participating in multidisciplinary care planning as necessary.
- Educate patients, family members, or representatives on rehabilitative care and activities promoting health, safety, and independent living.
- Accurately document observations, interventions, and evaluations using electronic medical records.
Qualifications:
- A current North Carolina Occupational Therapist license.
- Prior home care experience preferred but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience in electronic medical records (EMR) preferred.
