As a home care occupational therapist, you will be part of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to clients, allowing them to receive the necessary medical care to remain at home. Your primary responsibilities will include:
- Making home visits to clients in designated geographic territories
- Performing thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance
- Assessing cognitive, psychological, and motor aspects of performance and developing individualized intervention plans to achieve favorable outcomes
- Evaluating home environments for hazards or barriers to independent living
- Identifying equipment needs and environmental adaptations resulting in enhanced outcomes for clients
- Continually assessing and revising the occupational therapy care plan, participating with nursing in the multidisciplinary care plan as appropriate
- Providing education and instruction to patients, family members, or other patient representatives on rehabilitative care and activities necessary to promote health, safety, and independent living
