Occupational Therapist Job Description
We are seeking a skilled Occupational Therapist to join our team in Buckingham, Pennsylvania.
About the Role
The successful candidate will perform outpatient therapy evaluations and develop plans of care for patients in designated senior living communities or group homes. They will work closely with home health therapy partners to transition care from home health to outpatient services, achieving client-centered goals in their living environment.
Main Responsibilities:
- Perform evaluations, treatments, and proper discharge planning for clients using an outpatient EMR platform.
- Educate clients, family members, and caregivers on care plans to achieve a team approach to maximizing functional independence.
Requirements:
- A current PA Occupational Therapist license.
- Prior home care experience is preferred but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
- Solid computer skills and prior experience with electronic medical records (EMR) are preferred.
