BAYADA Home Health Care is seeking an experienced Occupational Therapist, OT, for a full-time opportunity performing outpatient home health visits. This role involves working with adult and geriatric patients on a per-visit basis in Independent and Assisted Living Communities throughout Monmouth County.

As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
Responsibilities:
- Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or in a group home setting for adults with intellectual disabilities.
- Professionally operate at top of licensure utilizing current research-based approaches to care in order to maximize functional independence for your clients.
- Work closely with your home health therapy partners to appropriately transition care from home health to outpatient services in order to achieve client-centered goals in their living environment, allowing the right care, at the right time for each client.
- Educate the client, family, and community caregivers on the care plans created for your clients in order to achieve a team approach to maximizing functional independence.
Qualifications:
- A current NJ Occupational Therapist license.
- Prior home care experience preferred, but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills, organizational skills, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
