About the Job
This full-time opportunity involves performing home health visits for our Gloucester County office, located in West Deptford, NJ. As a member of our multi-disciplinary healthcare team, you will provide rehabilitative care and skilled nursing to clients, allowing them to receive the medical care needed to remain at home.
Responsibilities
- Perform home health visits to provide physical therapy services to clients
- Collaborate with healthcare professionals to develop and implement client care plans
- Conduct assessments and evaluations to identify client needs and goals
- Develop and implement individualized treatment plans to achieve client goals
Requirements
- Graduation from a two-year college level program approved by the American Physical Therapy Association
- At least one year's work experience under the supervision of a qualified Physical Therapist
