* Provide one-on-one care to patients in their homes, including performing diagnostic tests and measurements, and implementing evidence-based interventions.
* Develop and maintain accurate records of patient interactions, treatments, and progress.
* Communicate effectively with healthcare teams to ensure seamless care delivery and coordinate interdisciplinary care planning.
Requirements
* Current licensure as a Physical Therapist (PT) in the state of DE.
* Previous home health experience required.
* Strong communication and interpersonal skills, with ability to build rapport with patients, families, and healthcare teams.
* Ability to travel throughout the region based on census.
