As a skilled Physical Therapist, you will be responsible for providing rehabilitative care and skilled nursing services to adult and geriatric clients in their homes.
Responsibilities
- Visit clients in designated geographic territories to assess and treat physical limitations.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Educate clients, family members, or representatives on rehabilitative care and activities necessary for independent living.
- Accurately document observations, interventions, and evaluations in client care management and services provided.
Qualifications
- Pennsylvania Physical Therapist license.
- Minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
