Key Responsibilities:
- Conduct comprehensive assessments of clients' physical function and mobility status
- Develop and implement personalized care plans to address specific needs and goals
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Provide education and training to clients and their families on proper techniques and exercises for optimal recovery
Requirements:
- Degree in Physical Therapy from an accredited institution
- Licensure as a Physical Therapist in Massachusetts
- Experience working in home health care or a related field (prior experience not required)
