Part-Time Physical Therapist - Home Health

You will be part of a multidisciplinary healthcare team providing skilled nursing and rehabilitative care to clients, ensuring their independence.
Responsibilities include:
- Performing diagnostic tests and measurements, such as mobility, strength, balance testing, and safety assessments.
- Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
- Continuously assessing and revising the Physical Therapy care plan, participating in the multidisciplinary care plan as needed.
- Educating clients and family members on rehabilitative care and activities necessary for independent living.
- Accurately documenting observations, interventions, and evaluations using a touch pad tablet.
Qualifications include:
- A current license as a Physical Therapist in Massachusetts.
