Job Description
We are seeking an experienced Physical Therapist to join our team in Aston, Pennsylvania. This is a full-time opportunity for home health visits with adult and geriatric clients.
You will be part of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing services to clients in their homes.
Your responsibilities will include:
- Conducting home visits to assess clients' needs and develop personalized care plans.
- Performing diagnostic tests and implementing interventions to promote wound repair, improve mobility, and enhance overall physical function.
- Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
- Collaborating with the multidisciplinary care team to ensure comprehensive care and revising care plans as needed.
- Educating clients, family members, or caregivers on rehabilitation techniques and promoting independent living.
- Maintaining accurate records of client interactions, interventions, and outcomes using electronic medical records.
To qualify for this position, you must have:
- A current Pennsylvania Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills and prior experience with electronic medical records (EMR) is preferred.
