Job Summary
We are seeking an experienced Physical Therapist to perform Outpatient home health visits for our senior living office in Exton, PA. This opportunity allows you to continue and grow your outpatient skills, spending more one-on-one time with patients and prioritizing quality over quantity.
About the Role
As a Physical Therapist at BAYADA Home Health Care, you will make outpatient visits to clients in designated geographic territories. Your responsibilities will include:
- Performing diagnostic tests and measurements, such as mobility, range of joints, transfer status, stability, and balance testing.
- Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
- Continually assessing and revising care plans, participating in multidisciplinary care planning as needed.
- Educating clients, family members, or representatives on rehabilitative care and activities necessary for independent living.
- Accurately documenting observations, interventions, and evaluations using electronic medical records.
Requirements
To be successful in this role, you will need:
- A current PA Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- The ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills and prior experience with electronic medical records (EMR).
