We are seeking a dedicated Physical Therapist to join our team in Bennington, Vermont.
The ideal candidate will have experience working with clients in their homes, creating personalized treatment plans, and using innovative techniques to help them reach their goals.
This role is part of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing services to clients, enabling them to receive the medical attention required to remain at home.
Responsibilities:
- Conduct home visits to assess client needs and develop individualized care plans
- Perform diagnostic tests and measurements, such as mobility and range of motion assessments
- Create and implement therapeutic exercises, gait training, balance, and other interventions to promote client health and independence
- Educate and instruct clients, family members, or caregivers on rehabilitative care and activities necessary for optimal health and safety
- Accurately document observations, interventions, and evaluations related to client care management
Qualifications:
- A current Physical Therapist license in the state of practice
- A minimum of one year of recent work experience as a Physical Therapist
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Solid computer skills and prior experience with electronic medical records (EMR) preferred
