To be successful as a Home Health Physical Therapist, you will need to have one year of prior clinical experience as a licensed Physical Therapist. You will work independently and manage your time effectively to provide rehabilitative care and skilled nursing to clients in their homes.
Responsibilities include:
- Conducting home visits to clients in designated geographic territories.
- Performing diagnostic tests and measurements to assess client mobility, strength, and balance.
- Developing and implementing individualized care plans to promote client health, safety, and independent living.
- Educating and instructing clients and their families on rehabilitative care and activities necessary for optimal health and wellness.
- Accurately documenting client care management and services provided using electronic medical records.
Qualifications include:
- A current Pennsylvania Physical Therapist license.
- One year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records preferred.
