BAYADA Home Health Care is seeking a skilled and compassionate Home Health Physical Therapist to join our team in Harrisonville, New Jersey.

The ideal candidate will possess a strong passion for providing high-quality patient care and have excellent communication skills. As a Home Health Physical Therapist, you will work closely with patients in their own homes to help them achieve optimal health and independence.
Responsibilities:
- Conduct home visits to assess patient needs and develop personalized treatment plans.
- Perform diagnostic tests and measurements, including mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Implement evidence-based interventions, such as manual therapeutic exercises, gait training, balance, and other therapies, to promote patient recovery and well-being.
- Educate patients and their families on rehabilitative care and activities necessary to promote healthy living.
- Accurately document patient progress and services provided using electronic medical records (EMR).
Qualifications:
- A current NJ Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with EMR preferred.
