The ideal candidate will have one year of recent work experience as a Physical Therapist and a current STATE Physical Therapist license. As a home care Physical Therapist, you will be part of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing to clients, allowing them to receive medical care at home.
Your responsibilities will include making home visits to clients, performing diagnostic tests and measurements, implementing integumentary protective interventions, developing individualized care plans, and educating clients on rehabilitative care and activities necessary for their health, safety, and independent living.
Qualifications
- A current STATE Physical Therapist license
- A minimum of one year of recent work experience as a Physical Therapist
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem-solving skills
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
