As a member of our Hudson County Visits team, you will provide home health visits to adult and geriatric clients on a per visit basis in designated territories throughout Hudson County.
This role requires at least one year of prior clinical experience as a licensed Physical Therapist. You will be an integral part of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing services to clients, enabling them to receive medical care required to remain at home.
Responsibilities include:
- Conduct home visits to clients in designated geographic areas.
- Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Implement protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assess and revise the Physical Therapy care plan, participating with nursing in the multidisciplinary care plan as necessary.
- Educate and instruct clients, family members, or client representatives on rehabilitative care and activities promoting the client's health, safety, and independent living.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
Qualifications include:
- A current NJ Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
